Pediatric Eyes and Smiles is proud to support local youth sports teams, school clubs, and community organizations whenever possible.We receive many requests throughout the year and carefully review each submission to ensure alignment with our mission of supporting children, families, and community wellness. Sponsorship requests are reviewed quarterly, and priority is given to current Pediatric Eyes and Smiles patients and families. Due to the high volume of requests, support is limited and submission of this form does not guarantee approval.Please complete the form below to request sponsorship or support.
